These decisions about what is or is not a mental illness have rarely been grounded in science. To some degree it’s because the NIH has no interest in funding science that answers important real world impact. They are mostly funding the research and development of profitable drugs.
There has been great reporting done on how homosexuality was removed from the list of mental illnesses in the 70s. This is a fine example of a broken clock being correct twice a day. Yes it was correct to normalize homosexuality. But that decision was not made based on rigorous application of the scientific method. It was grounded in the fact that the some of the people in power were gay.
Much of what is or is not considered a mental illness is socially constructed and would be preferable if the psychiatrists stopped pretending this was about science.
The document termed the DSM (Diagnostic and Statistics Manual) which defines diagnoses is not a scientific document. It is written by a committee, and has a scientific direction. But the trannie normalization involved a politicized procedure in which trans wack activists pushed the interpretation that they wanted. Essentially, the trans state was defined as a normal state. Of course, this means that if you are trans there is nothing wrong with you (false). For the insurance perspective this is a problem - how then could treatment be covered, if it was normal? The solution is to define DISTRESS as the state needing treatment. This opened the door for the insanity of the treatment situation - there is no objective evaluation going on, it's all about distress and anxiety and depression.

We now know there are real neuronal differences in trans people, though it can only be examined after death. That's false. The brain studies are all garbage. The sample sizes are way too small (very low power) and they use cherry-picked regions which never replicate. Robert Sapolsky is a known crank.
