Believe it or not there are a small number of adults with genuine gender dysphoria who actually do benefit from transition surgery. What happens to them if you drive all the specialist surgeons away? BTW I am totally against evidence-free transitioning of minors but nor should you throw the baby out with the bathwater.
It is preposterous to think that the mutilation of healthy bodies is an answer to a psychological condition, especially given the inevitable side effects and complications that result. “Gender Reassignment Surgery” is a clear violation of the Hippocratic Oath’s insistence that physicians do no harm. It is by definition harmful: intervening in a complex system (the human body) in ways that no physician fully understands or whose negative consequences they can predict. So no, it is NEVER wise to do so. Rather, what such people people need is psychological help to learn to accept the sex they are, which cannot be changed because it defines them on a cellular level.
Exactly! Pretending to be the opposite sex is a maladaptive coping mechanism for a mental problem. Pretending never solves the problem, it covers it up and it festers.
Please explain to me the medically verifiable way to diagnose a “genuine” illness (birth defect? What would you call it?) which requires invasive and dangerous cross-sex hormones and surgery in order to mimic the opposite sex.
The tone of your question suggests to me that you have a highly polarised view of transitioning therapies in all cases.
Firstly I will stress again that I am AGAINST any transition therapy in minors because
1. The alleged dysphoria is grossly over diagnosed and has become something of a fad, thanks to social media
2. The quality of evidence supporting it in minors is very poor.
In adults, once you weed out the transvestites etc, there are a small number with true gender dysphoria, as recognised by WHO and indeed it’s in the DSM-5, although what’s listed there are the mental health issues caused by gender dysphoria, not suggesting that the dysphoria itself is a mental illness.
In these adults there are a range of treatment options which don’t require hormones or surgery, but in a small number of cases gender reassignment is the most effective way to improve their mental wellbeing. A detailed review was published by Nobili et al in 2018 and you will find a detailed review of the main studies on this issue published by the NIH in 2019.
It’s important when you argue against gender reassignment that you don’t throw the baby out with the bath water.
In a small number of cases… so how can you then say to not throw the baby out with the bath water?
Let’s go back to normality, to common sense, that is no gender reassignment, no meds (pharmacy), get it out of the media and then help the small number of cases outside the view of the big microscope of media and society.
That’s exactly what was happening until social media came along, identitarianism flourished and gender “dysphoria” became a fashionable fad, sucking in large swathes of the medical profession. So now there’s a tsunami of bathwater surrounding one small baby. So tell me how to fix it but still protect the baby.
Actually, there's a tsunami of otherwise healthy babies surrounding an unhealthy one. Responsible adults don't expose the healthy ones to the illness.
Psychogenic illness spreads because it's irresponsibly put into the minds of vulnerable ppl. Of course 'transitioning' helps ppl who hear about it & fixate on it as a solution to their distress. If it wasn't an option, there would be no fixation, and other solutions would be sought. 'Gender transitioning' surgeries & drugs are as unethical as encouraging BIID and then cutting off the limbs of the ppl who then manifest BIID. Or encouraging anorexics to get bariatric surgery. It's barbaric and should be illegal in civilized countries. If a tiny minority of people go elsewhere to get sexually mutilated, so what? Making such mutilation illegal would protect the vast majority who will figure out another way to thrive without doing so.
I would argue we go back to conservatism, traditional values, normality, common sense. Comparable to countries like Russia, China, Hungary, Iran and others (yes, the so called axis of evil, except for Hungary).
(A pipe dream)
However, there will always be a ‘wealthy’ minority that is opposing such change as I am convinced that most people don’t care who is in charge as long as they can live their lives and feel secure.
The babies are collateral damage. Just bad luck, like others that have misfortune in their lives.
Same argument Adults can fly to Thailand. Adults can work it out for themselves they’ll find a practitioner willing to do their “job” if they must have it!
WPATH did a systematic review of surgical outcomes (Baker et al is part of the many reviews they commissioned), finished it, and then interfered with their publication. Suspect that it didn't show the interventions were as beneficial as they hoped, but do not know. The protocol says the reviews were completed. The HHS report talks about this in its WPATH section.
Does Nobili give the certainty of the body of evidence, the last step for a systematic review, after synthesizing? I see a paper led by Nobili from that year on quality of life that says: "The majority of these articles displayed either high or moderate risk of bias." Only 2 studies about quality of lif were low risk of bias and those seem cross sectional, so you can't attribute cause and effect to them? I also couldn't see how long after treatment they were considering the patients, there are honeymoon periods for a lot of these treatments (for example, regret and detransition seem to happen at a median of 8 years or so in some of the older surgical studies)....?
I think there is great uncertainty about likely outcomes for these interventions, although there is a huge ramping up in providing them, a growing market that surgeons are very excited about, judging from the advertised trainings, but they are not following up!
No psychiatric diagnosis, of adults or children, can be verified by a neurochemical or neuroanatomical test. That's why psychiatry is not neurology. Psychiatrists control disapproved behaviors; neurologists treat diseases. (Many neurologists abandon science to embrace psychiatry, it is true.) Lobotomy was invented by a neurologist. He received the Nobel Prize for that, and it has never been rescinded.
mastectomy for transitioning females would constitute less than 1% of their work. Loss of the even smaller amount in minors will not drive them away but may make them think more carefully even in adults.
If an adult feels a strong desire to emulate women, and can find a quack who will mutilate his body, then more power to him. But not children.
The crock of "gender dysphoria" is part of the much larger psychiatric package which leads to the medicalization of many childhood behaviors having nothing to do with physical abnormalities; which is to say non-diseases. All of those treatments should be abolished.
Whoa! Men get to do anything they want that doesn’t impinge upon your privacy or safety, just as some lesbians get to emulate men. The idea that merely play acting as female oppresses you is your problem.
Drag shows have been around for who knows how long. Have they oppressed you? Are you going to do a Carrie Nation on them?
Privacy or safety: you get those and are entitled to nothing more. You are not entitled to not be offended, or to project culpability onto people who aren’t directly impinging on your privacy or safety, which are not abstractions like “oppression” and “dignity.”
Yes, adult men can dress up and buy fake tits and genitals entirely without regard to your feelings. They are also entitled to the privacy and safety to which you lay claim.
Inside of 5 minutes I was accused of being a lefty and a righty. Reductionism obviates thought, hence you made no attempt to disprove my comment. In another post you accused someone of voting for Trump, so you are basically reduced to talking points and insults.
I can remember a time when the Left vociferously objected to genital mutilation. Now it vociferously insists that it should be done to children.
You are a physician, according to your profile. Do you think that lobotomy is a legitimate medical treatment? It did win the Nobel Prize for its inventor Moniz.
You will note that I do attempt to engage in civil argument where possible - until people start making insane and insulting comments. Then I will call them out for what they are.
Medicine is far from perfect and has made many mistakes. But like any good science, hopefully it learns from them and improves. But its touchstone should always be caring for people with as much compassion as possible, while keeping the available evidence in mind. Political ideology has no place here.
You didn't call out a Trump voter, you said someone probably voted for Trump who then denied it. So, let's not pretend that you are the White Knight of civility. You then falsely suggested that I'm conservative, so your instincts are quite defective. You are the one who twice introduced ideology where it had not appeared. So, own the. hypocrisy.
There are men whose desire to live out their male fantasy of what it is to be a woman is encouraged and financed by male-dominated society, against the dignity, privacy, and safety of girls and women.
And there are women trying to escape the violent oppression of male-dominated society.
NO ONE benefits from experimental elective cosmetic drugs and surgeries to destroy their bodies for the sake of sexism.
Key word in your retort: ADULTS. If it's just adults getting their bodies multilated, no problem. But don't bother with "they can only mutilate adulrs if they learn on kids" ...
Not sure what you’re on about. Read what I said about kids. And using the word “mutilate” for adults displays a degree of ignorance and a total lack of compassion.
Using the word "mutilate" reflects my total and complete understanding but no doubt for "compassion" I am expected to regurgitate some tripe like "gender affirming care"
Some justification for that though there's the nontrivial problem of those "transsexuals" who insist they've changed sex and demand to be treated as if they had done so.
Agreed. The beginning of the end. “The case did not ask jurors to decide broad ideological questions about gender medicine. Instead, it focused narrowly on whether these specific clinicians exercised appropriate medical judgment in this particular case. The jury concluded they did not.” It would be great to find out what the psychologist wrote up regarding the mutilation justification of this victim as well as the other evidence.
It's about goddamned time. A few years ago, I predicted it would be the lawsuits, ensuing from detransitioners that had been swept up in a social contagion as teens, that would put an end to this lunacy. But I was more pessimistic and thought it might take another 10 years for the tide to turn. More of this please.
Statutes of limitations also need increased to give young ppl more time to come to terms with what happened to them and hold 'professionals' accountable.
The medical societies have refused to budge, the (even highly regarded) medical journals keep publishing incorrect papers, blocking corrections and blocking correct papers, and the NYT keeps framing it as left/right, the HHS report as non-evidence based and political, etc etc....The lies continue. They won't budge. Any of them could stop it.
The harm has been increasing, the gender MD's are gleefully training up more and more surgeons to remove breasts and genitals from young men and women and girls and boys. Check out what is listed as covered in their conferences.... It's such a big lie that no one will believe it could be happening. Surely you would not do this to young people if it was so serious and so poorly understood, right? Surely anyone who told you there is a problem would just be lost, because otherwise, it would be *crazy*!
The hospitals are telling the patients that they are being forced to not provide this "medically necessary" "care"--the patients and their families and the public and the attorneys general (looking at you, here in my state!) all have no idea, do not believe there is anything questionable. They think it is well understood as a condition, that the treatment is supported by evidence, that it is likely to be beneficial and that yes, not getting these interventions is harmful. They are completely uniformed.
Some people have told me for years that you can produce systematic reviews and evidence and rebuttals til you are blue in the face, careful articles on what has happened, what is happening (by reporters, e.g., here and by Ryan, Singal, Selin Davis, Block, many others, etc etc), (yup, here I am!), and what will move it is lawsuits.
And here we are:
"For years, critics of pediatric gender medicine have warned that vulnerable minors were being rushed toward irreversible interventions without science-based psychological exploration, without meaningful long-term outcome data, and under an atmosphere of emotional coercion that routinely invoked suicide risk to justify speed. In this case, jurors heard testimony that a parent opposed the surgery but consented out of fear that refusal might lead to self-harm. They heard evidence that clinicians lacked full knowledge of the patient’s doubts and psychological history. They heard that key records were never obtained and that the professionals involved failed to meaningfully consult with one another.
In other words, they heard facts that successful trial lawyers understand very well.
This verdict is not the end of the story. It is the beginning."
It is not a day too soon.
I hope to read your book on it someday soon too--thank you for digging into this!
I am clinician working with young GD people and their families, also an academic and researcher who is widely published on the subject but have been systematically ignored and criticized by the health professional community.
People that mouth that mantra need to be more seriously challenged with the question, "why?" Of course it only takes most of them about 30 seconds to call the questioner a moron rather than offer any reasoning or argument for their mantra at all.
Yes! Indeed, John. Regulation, especially self-regulation, is a total failure because humans in groups are so susceptible to moral narcissism, social contagion, and groupthink.
I agree! Daniel Biss, mayor of Evanston, is running for Congress in the 9th district. I was horrified to learn at a recent gathering that this otherwise upright and inspiring individual, is 100% for gender surgery for minors, having allowed his two sons to transition that way. I wrote him a note but am going to write again, sharing your post. I agree that people will look back on this era as akin to the era of lobotomies.
Update: The mayor wrote back to me to say he was grateful I shared Gerald Posner's post. And I think he really was too!
If Mr Biss was as upright and inspiring as you say he would have protected his children. He failed his own flesh and blood on a very fundamental level and shouldn’t be trusted on any other issue
The Prime Minister of Canada is reported to have a trans offspring too so good luck getting anything looked at in Canada. Getting rid of these ideologically corrupted politicians is nevessary.
This is good news, but be aware that the Transqueer Party is very likely to fight back with new GAC-provider indemnity laws in the states it controls (aka "blue" states), as well as strict enforcement of existing Transqueer Party laws, e.g.,
>In California, you have the right to receive medically necessary gender-affirming care or any other medically necessary healthcare without discrimination. Insurance companies and health plans cannot deny coverage or benefits based on a person’s gender identity or diagnosis of gender dysphoria. Nor can they deny coverage because of inconvenience or inadequate provider networks. If no in-network provider is available within the required time or distance, your insurer must arrange care with an out-of-network provider. Insurance companies also cannot penalize or discriminate against doctors for providing gender-affirming care.
The bottom line is that we cannot ultimately rely on the courts. To succeed, it is absolutely necessary to disempower the Transqueer Party wherever and whenever we can, at all levels of government, from school board to POTUS.
Shrinking from that inconvenient truth will only delay the end of the still ongoing crimes against humanity that the Transqueer Party fully empowers and supports.
The cost of medical insurance for not just doctors , but for institutions involved in "gender identity' should seriously slow this plague. It's going to have to be done case by case where each situation has to be evaluated separately on their merits. I wish there could be a sweeping judgement , but that's not how this works. But, one huge claim should send a warning to the insurance industry.
Hopefully there will be suits brought against schools that refused to alert parents to identity change of their child, or in California actually forbid it. It will be the pain of individual suits and then the difficulty of getting insurance coverage that will have the most impact in stopping this.
It would make sense to me to put energy into informing insurance companies of what is likely to be a changing landscape and how this put's them at financial risk regarding who they insure. If there's to be an educational outreach , this would be in industry to target.
I suspect the insurance industry was watching this closely. After Skrmetti, DT’s executive orders and the rash of subsequent clinic closures, it has to be on their radar.
Excellent news, as what occurred was legalized medical malpractice, and anyone with a conscience knows it, including the parents. The government, parents and medical society have abdicated their responsibility. Money won’t fix the evil done, and the only solution is mankind turning to Christ, to heal the brokenness caused by the evil in the hearts and minds of mankind. This is clear evidence of turning away from God.
Large awards for malpractice is the only way to stop these so-called "doctors". Hitting them in the pocketbook is the only way to change their behavior.
This is not about adult transitioners. Please do not conflate adults with minors in this debate. This judgement was about medical malpractice causing irreversible harm to a minor, who could not give consent. The parent was emotionally blackmailed by the threat of suicide to reverse their objection.
Technical, numerological “adulthood” these days is meaningless. The average age of first) marriage may be the clue as to when functional adulthood arrives in this dysfunctional culture. So that’s about 28-30
If I’m a doctor specializing in trans surgery I’d be looking for another area of specialty to transition to 😏

Just don't come back to America demanding to be able to violate girls' and women's privacy, dignity, and safety just because you flew to Thailand.
Solved.
They are either Breast Surgeons or Plastic Surgeons. I don’t think they will be joining the unemployment queues.
The question I asked is what happens to the PATIENTS…
Well, I just saw a panel of four, 20 something "transitioned" former females discussing this very topic, and they are pretty pissed off!
Great sample size. Just a little better than an anecdote.

Men don't get to "identify" as women. They don't get to violate our privacy, dignity, and safety.
Oppressors don't get to "identify" as the people they oppress. Everyone knows that's disgusting and WRONG.

Such closed-mindedness. It’s become a real feature of the conservative movement’s inexorable move to the right.
The fight against the "transgenderism" gender-enforcement lobby is coming from the left.
TERFs have been fighting this fight for far longer than the right.
And we're going to win. Because women's rights are not up for grabs.

Come on. Stop dissing the "warmth of collectivism."
What are the odds that both sons spontaneously suffered from gender dysphoria? Some parental suggestion must have been involved.
I totally agree! I decided not to trust his judgment on anything else even if he seems upright and inspiring. I think he's blind rather than evil.
Ideologically captured and emotionally blackmailed!
In that case I suspect Mr. Biss is beyond reaching (by anyone except maybe Jesus Himself - but how does one repent of such failings? Lord have mercy).
The Prime Minister of Canada is reported to have a trans offspring too so good luck getting anything looked at in Canada. Getting rid of these ideologically corrupted politicians is nevessary.
The mayor wrote back to me after I shared Gerald Posner's post. Here's what he said:

Thank you for sharing this -- I really appreciate it.

Hence why the insurance companies are fleeing California and other “Blue” states. Legislation passed by fools who can’t perceive their own downfall!
Good idea!
I suspect the insurance industry was watching this closely. After Skrmetti, DT’s executive orders and the rash of subsequent clinic closures, it has to be on their radar.
I have learned it’s best never to assume anything.
They already know they constantly monitor and model these issues.
This is clear evidence of turning away from God.
will be interesting to see how this affects plastic surgeons in the US and world wide
Large awards for malpractice is the only way to stop these so-called "doctors". Hitting them in the pocketbook is the only way to change their behavior.
Thank you for informing us of this crucial win for minors who are victims of irreversible pediatric gender surgeries. 🙏
And medical therapies that harm the brain and body irreparably.
Thank you! This article gives a glimmer of hope in one of the darkest periods of history and will hopefully signal the beginning of the end.
